Baptism Information & Questionnaire


If you are interested in baptism, please read and complete the following. You need to watch the Basic 1 class on baptism or schedule and attend a baptism class before being baptized.

This Questionnaire is for adults considering baptism. (For those below age 18, Parent should contact the appropriate Age Group Minister/Director for information about completing a baptism class. In some instances a family may attend the in-person baptism class together, call the church office to talk with Pastor Tracy Johnson to schedule this.)

At Immanuel we are committed to and practice Believer’sBaptism, pictured by immersion.

Our Basic 1 class on Baptism will help you understand this more, and completion is required before being baptized. The following will help us get to know you better and prepare you for the Basic 1 class.

Water Baptism is an outward sign of an inward change.


- It is an obedient response to God and an expression of your commitment to Christ.


- It is a way to show others that you have entered into a personal relationship with Jesus Christ and symbolizes what took place in your heart at the time of salvation.


- Because Jesus instructed us to be baptized, baptism is an act of obedience. While it is not necessary for salvation, Water Baptism demonstrates your submission to God and commitment to live for Christ.


- Water baptism by immersion pictures what Christ has done for us--He came, suffered, died for our sin, was buried, arose and is alive now and will at the appointed time return in triumph.


- When you accepted Jesus Christ as Lord and savior, you shared in his death and resurrection, which is symbolically illustrated by going under the water (dead to your old life) and rising again (new life).

To help as a guide in preparing for Believer’s Baptism, please share some of your story of how you came to know Jesus Christ as your personal Lord and Savior.

First, here are some basic Bible verses that help explain WHY one is baptized:
• Romans 3:23-- for all have sinned and fall short of the glory of God
• Romans 6:23-- For the wages of sin is death, but the free gift of God is eternal life in Christ Jesus our Lord
• Romans 5:8-- but God shows his love for us in that while we were still sinners, Christ died for us.
• John 3:16-- For God so loved the world,[a] that he gave his only Son, that whoever believes in him should not perish but have eternal life.
• John 1:12-- But to all who did receive him, who believed in his name, he gave the right to become children of God, (All verses from the English Standard Version of the Bible)
• A person has personally realized they are a sinner and they ask God to forgive them of their sins, and the invite Jesus Christ (God’s only unique son) to be their Lord/King and Savior.

Second, after one has received Christ Jesus as Lord and Savior, they follow thru with Believer’s Baptism as a testimony of receiving God’s gift of salvation, forgiveness and eternal life, and they desire to publicly declare this before the Church in the act of baptism.
